  • Full Name (including middle initial)
  • Date of Birth
  • Current Address and Phone Number
  • Purpose of the Appointment
  • Current Insurance Information
  • Current Employer's Name & Phone

It is important to let the scheduler know about ALL of the concerns you would like addresses so that we can schedule the appropriate amount of time to care for your needs.

Requesting a Referral

Many health plans require referrals to specialty physicians or other health care providers for services that primary care providers are not able to provide. To request a referral, follow the steps below:

  1. Visit Your Primary Care Provider
    Your physician (or other provider) will evaluate your problem and make a referral to a specialist if necessary. If your health plan requires you to see someone from a list of preferred specialist, please bring that information with you to your appointment.
  2. Verify Your Insurance and Referral Information
    Double check with your insurance provider that the physician you have been referred to is a preferred provider and that your visit will be covered. Make sure our receptionists have the proper insurance information so that we can send your referral request to the correct plan without delay.
  3. Make an Appointment with the Specialist
    Following confirmation from your health plan that your referral has been approved, you may call and make your appointment with the specialist. Some specialities require the referring provider to book the first appointment. If this is the case, your provider will work out the details with you. You may want to verify with your primary care provider that any pertinent medial records have been sent to the specialist ahead of time.
